How to Complete Background Checks
Arizona Level One Fingerprint Card
Of the 8 required background checks, the Arizona Level One Fingerprint Clearance Card meets the requirements for 5 of the checks.
- National FBI Fingerprint Criminal History
- In-State Criminal History
- National Crime Information Center National Sex Offender Registry
- Arizona Sex Offender Registry
- Previous States of Residence Sex Offender Registry
To apply for your Arizona Level One Fingerprint Clearance Card, go to https://www.azdps.gov/services/public/fingerprint.
Click on Apply for a Card, then apply for a Fingerprint Clearance Card.
Arizona Child Abuse and Neglect Registry Check
DHS Licensed Center or Group Home (not contracted with DES)
To request this check providers must submit a Direct Service Central Registry Clearance Form (DCS-1083B) to DCSCentralRegistry@azdcs.gov.department form as described at https://www.azdhs.gov/licensing/childcare-facilities/index.php#providers-non-contractor-registry
The following types of providers are required to complete a Direct Service Central Registry Clearance Form (DCS-1083B) for each employee or Household Member over the age of 18 and email it directly to Department of Child Safety using the following email address: DESCANRegistrychecks@azdcs.gov in order to initiate a central registry background check:
Interstate Child Abuse and Neglect Registry Check
This check is only required if the provider, employee, or household member has lived in another state within the last 5 years. To request an interstate child abuse and neglect registry check, complete the Request for Search of Background Checks Form (CCA-1211A). You will need to download and complete the State Specific Forms prior to submitting your request.
NOTE: If you are a DHS Licensed Center or Group Home that is not contracted with DES, Arizona will provide information regarding completion of this check, and share here once finalized.
